Parents should download and complete the forms below to provide the school with information about their child.
All applicable forms should be completed and emailed to cmcateer173@c2kni.net
In the subject bar of your email, please state your child's name followed by the words 'Data Collection Forms'.
Only P1 parents are required to complete the 'Additional Needs Questionnaire'

This form provides the school with all the necessary contact, background and medical information about your child.

Pupil Collection Information
The school requires this information so the teachers know who has permission to collect your child from school.

Child Protection and Photograph Permission Form
This form makes you aware of our Child Protection procedures and relevant safeguarding policies. It is also used for parental permission for local visits and the use of your child's photographs.

This questionnaire should be completed by all new P1 parents and it provides the school with information of any additional needs your child might have.
